Roles & Responsibilities

• Evaluate patients’ functional status and create individualized OT treatment plans
• Deliver hands-on and adaptive interventions to restore independence
• Set meaningful, measurable goals in collaboration with patients and families
• Document progress clearly and timely to support continuity of care
• Collaborate with interdisciplinary team members to optimize outcomes
• Educate patients, families and caregivers on home strategies and adaptive equipment

Qualifications

• 2+ years of Occupational Therapy experience (as noted in job posting)
• Active Occupational Therapy license or ability to obtain state licensure for assignment
• Strong clinical assessment and treatment planning skills
• Comfortable with documentation in an acute/rehab setting and meeting productivity expectations
• Excellent communication and teamwork skills; patient-centered attitude
• Willingness to complete required onboarding and facility-specific training

Education

• Master’s degree or OTD in Occupational Therapy (or equivalent entry-level degree in OT)

Knowledge & Skills

• Skilled in ADL/IADL assessment and intervention
• Knowledge of splinting, therapeutic modalities and adaptive equipment
• Familiarity with discharge planning and continuity of care processes
• Proficient documentation and charting practices
• Ability to work collaboratively within an interdisciplinary rehab team
