Roles & Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans that promote independence
• Provide targeted therapy for activities of daily living, mobility, and functional skills
• Implement therapeutic exercises and activities to improve coordination and upper extremity function
• Monitor resident progress and adjust treatment plans based on outcomes and clinical judgment
• Collaborate closely with physicians, therapists, nurses, and interdisciplinary team members to coordinate care
• Maintain accurate and timely clinical documentation to support quality care and regulatory compliance
• Educate residents and caregivers on adaptive techniques and safety strategies to boost confidence and independence

Qualifications

• Active California Occupational Therapist license required
• Degree in Occupational Therapy from an accredited program required
• Previous Skilled Nursing Facility, rehabilitation, or geriatric therapy experience preferred
• Strong clinical assessment and patient care skills with the ability to create practical treatment plans
• Excellent communication and documentation abilities and a team-oriented mindset

Education

• Degree in Occupational Therapy from an accredited program

Knowledge & Skills

• Expertise in ADL training and functional rehabilitation
• Strong patient evaluation and treatment planning skills
• Clinical assessment, therapeutic exercise, and upper extremity rehabilitation knowledge
• Effective communication, interdisciplinary collaboration, and accurate documentation skills
• Ability to educate residents and caregivers on adaptive strategies and safety
