Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in the Mercy LIFE program
• Administer medications and treatments accurately and in accordance with policies
• Monitor and document vital signs, clinical changes, and resident responses
• Perform and document required Trinity specialty competency tasks and skills checklist
• Collaborate with interdisciplinary team members to support individualized care plans
• Maintain strict infection control, safety, and documentation standards
• Communicate effectively with residents, families, and staff to ensure continuity of care
• Complete required health screenings, drug screens, and vaccination documentation promptly

Qualifications

• Current Licensed Practical Nurse (LPN/LVN) credential — active and verifiable
• Basic Life Support (BLS) certification required
• Advanced Cardiac Life Support (ACLS) certification required
• Successful completion of License/Certificate Verification and Trinity Specialty Specific Competency Testing
• Ability to pass Trinity Color Vision, 2-step PPD or IGRA (Quantiferon), and Trinity 10-panel drug screen
• Up-to-date immunizations: MMR, Hepatitis B, Varicella, Tdap, COVID-19, and annual influenza as required
• Comfortable working in a per diem/pool model and adapting to varying schedules

Education

• Completion of an accredited practical nursing (LPN/LVN) program or equivalent

Knowledge & Skills

• Strong clinical assessment and medication administration skills
• Accurate documentation and familiarity with electronic health records
• Skilled in infection control and standard precautions
• Good critical thinking and the ability to escalate changes in condition appropriately
• Effective communication and teamwork with interdisciplinary care teams
• Ability to complete competency-based checklists and specialty testing
