Roles & Responsibilities

• Provide compassionate, professional nursing care to residents in the Trinity Health LIFE program
• Administer medications and treatments accurately and safely according to policy
• Monitor and document patient conditions, reporting changes to the care team promptly
• Assist with care planning and coordinate with interdisciplinary team members
• Perform routine procedures and basic wound care per facility protocols
• Maintain clear, timely charting in the electronic medical record
• Support resident dignity and independence through patient-centered care
• Adhere to infection control, safety standards, and facility policies at all times

Qualifications

• Current LPN license valid in New Jersey
• Completion of an accredited LPN program
• Current BLS (Basic Life Support) certification preferred or required upon hire
• Must meet Trinity Health health requirements: MMR, Hepatitis B, Varicella, Tdap, Physical/Health Statement, COVID-19 vaccine, Influenza vaccine
• Successful completion of Trinity 10-panel drug screen and required background checks
• Strong communication skills and the ability to collaborate with a multidisciplinary team
• Prior experience in long-term care, community health, or similar settings is a plus

Education

• Completion of an accredited Licensed Practical Nurse (LPN) program; high school diploma or equivalent required

Knowledge & Skills

• Proficient in medication administration and basic nursing procedures
• Familiarity with resident assessment and documentation practices
• Ability to follow care plans and implement physician/nurse directives
• Good clinical judgment, prioritization, and time management
• Comfortable using electronic medical records and standard clinical technology
• Compassionate bedside manner and ability to engage with older adults
