Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents and patients
• Administer medications and treatments safely and accurately
• Monitor and document patient status, vital signs and changes in condition
• Collaborate with the interdisciplinary team to create and follow individualized care plans
• Assist residents with activities of daily living and promote independence
• Maintain infection control standards and a safe care environment
• Communicate clearly and empathetically with patients, families and staff
• Participate in shift handoffs and documentation to ensure continuity of care

Qualifications

• Active LPN licensure required (Pennsylvania) or eligibility to obtain licensure
• Current BLS/CPR certification or willingness to obtain upon hire
• Ability to pass background check and required health screenings
• Strong communication skills and a team-first mindset
• Previous LPN experience in long-term care or clinical setting preferred

Education

• Graduate of an accredited practical nursing program
• High school diploma or equivalent

Knowledge & Skills

• Medication administration and safe documentation practices
• Vital signs measurement and basic patient assessment
• Electronic medical record documentation familiarity
• Basic wound care and infection prevention techniques
• Time management and prioritization in a fast-paced environment
• Compassionate resident care and effective interpersonal skills
• Ability to follow care plans and escalate changes appropriately
