Roles & Responsibilities

Provide compassionate bedside care to residents in a skilled nursing setting
Administer medications and treatments accurately and document appropriately
Monitor vital signs, report changes, and collaborate with the care team
Assist with activities of daily living while promoting resident dignity
Communicate effectively with residents, families, and interdisciplinary staff
Participate in care planning and ensure documentation meets facility standards

Qualifications

Valid LPN (Licensed Practical Nurse) license in Pennsylvania required
Completion of an accredited LPN/LVN program
Current BLS/CPR certification (or obtain upon hire per facility policy)
Comfortable with medication administration, wound care basics, and vital sign monitoring
Strong communication skills and a compassionate approach to resident care
Ability to pass facility background check and drug screening as required

Education

Graduation from an accredited Practical Nursing (LPN/LVN) program

Knowledge & Skills

Fundamentals of nursing practice in skilled nursing/long-term care
Medication administration and accurate charting
Basic assessment skills and recognizing changes in condition
Infection control and safe patient handling techniques
Teamwork, time management, and patient-centered communication
