Roles & Responsibilities

  • Provide compassionate hands-on nursing care to assigned patients
  • Administer medications and treatments accurately and on schedule
  • Perform routine assessments and document patient status clearly
  • Collaborate with RNs and interdisciplinary team to deliver coordinated care
  • Support patient comfort, safety, and personal care needs
  • Follow facility policies and maintain compliance with clinical standards

Qualifications

  • Active LPN license valid in New York (required)
  • Recent hands-on LPN experience in a long-term care or skilled nursing setting preferred
  • Basic Life Support (BLS/CPR) certification or willingness to obtain
  • Strong communication skills and a patient-centered attitude
  • Ability to pass required background checks and health screenings

Education

  • Graduate of an accredited LPN/LVN program or equivalent diploma/certificate

Knowledge & Skills

  • Medication administration and documentation best practices
  • Basic wound care and infection control procedures
  • Vital signs assessment and routine patient monitoring
  • Familiarity with electronic charting and clinical documentation
  • Time management, teamwork, and patient advocacy skills
