Roles & Responsibilities

Provide compassionate hands-on nursing care to assigned patients
Administer medications and treatments accurately and on schedule
Perform routine assessments and document patient status clearly
Collaborate with RNs and interdisciplinary team to deliver coordinated care
Support patient comfort, safety, and personal care needs
Follow facility policies and maintain compliance with clinical standards

Qualifications

Active LPN license valid in New York (required)
Recent hands-on LPN experience in a long-term care or skilled nursing setting preferred
Basic Life Support (BLS/CPR) certification or willingness to obtain
Strong communication skills and a patient-centered attitude
Ability to pass required background checks and health screenings

Education

Graduate of an accredited LPN/LVN program or equivalent diploma/certificate

Knowledge & Skills

Medication administration and documentation best practices
Basic wound care and infection control procedures
Vital signs assessment and routine patient monitoring
Familiarity with electronic charting and clinical documentation
Time management, teamwork, and patient advocacy skills
