Roles & Responsibilities

• Provide compassionate care to residents in a skilled nursing facility
• Administer medications and treatments as prescribed
• Monitor patient vital signs and report changes to the healthcare team
• Collaborate with nurses and aides to ensure high-quality care
• Document patient care accurately and timely

Qualifications

• Current and valid LPN license in New York State
• Strong clinical assessment and critical thinking skills
• Excellent communication and teamwork abilities
• Ability to adapt to a fast-paced environment
• COVID-19 and Influenza vaccines required (per facility policy)

Knowledge & Skills

• Knowledge of long-term care and skilled nursing regulations
• Proficient in medication administration and wound care
• Comfortable using electronic health records (EHR) systems
• Compassionate patient-centered approach
