Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents following care plans
• Administer medications and treatments accurately and on schedule
• Monitor and document vital signs, patient responses, and clinical changes
• Communicate effectively with RNs, physicians, and interdisciplinary team members
• Assist residents with activities of daily living while promoting dignity and independence
• Report changes in condition promptly and participate in care planning
• Maintain accurate and timely clinical documentation in the patient record
• Adhere to infection control practices and safety protocols at all times

Qualifications

• Current, active LPN license in Pennsylvania
• Basic Life Support (BLS) certification required or willing to obtain
• Comfortable with night shift routines and able to work independently
• Background check and drug screening clearance as required
• Prior experience in long-term care, rehab, or med-surg settings is a plus

Education

• Completion of an accredited Practical Nursing program (LPN/LVN diploma or certificate)

Knowledge & Skills

• Strong medication administration and documentation skills
• Patient assessment and basic clinical judgment
• Clear, compassionate communication with patients and team members
• Familiarity with electronic charting and bedside documentation
• Infection control, safety procedures, and resident-centered care principles
• Time management skills to balance charting, meds, and direct care
