Roles & Responsibilities

Deliver compassionate hands-on care to patients in home health settings
Administer medications and treatments with precision and empathy
Monitor patient progress and collaborate with the care team for optimal outcomes
Educate patients and families to empower their health journey

Qualifications

Active LPN license in good standing
Current BLS certification
Experience in home health or similar settings preferred

Education

Completion of accredited LPN program

Knowledge & Skills

Strong clinical assessment abilities
Excellent communication and patient rapport skills
Proficiency in medication administration and wound care
