Roles & Responsibilities

• Provide compassionate, hands-on personal care and assistance with activities of daily living
• Assist with patient transfers, mobility and safe positioning to support rehabilitation goals
• Monitor and report changes in patient condition promptly to the nursing team
• Document care and observations accurately and timely in the facility charts
• Collaborate with nurses, therapists and family members to deliver patient-centered care
• Support facility routines including meal assistance, hygiene, and therapeutic activities

Qualifications

• Current LNA / Licensed Nursing Assistant certification (eligible to work in Vermont)
• Proven experience as an LNA or nursing assistant preferred
• Reliable transportation — travel assignment requires local mobility
• Weekend availability — every other weekend required
• Willingness to work evening shifts (3:00 PM - 11:00 PM) and occasional overtime
• Ability to pass background check, drug screen and required immunizations

Education

• High school diploma or GED required; completion of an approved nursing assistant training program preferred

Knowledge & Skills

• Strong patient care and bedside manner — kindness and patience go a long way
• Basic clinical skills: vital signs, ADL assistance, safe transfer techniques
• Clear, professional communication and teamwork
• Good documentation and charting habits
• Infection control practices and adherence to facility procedures
• Time management and flexibility in a fast-paced rehab/long-term care setting
