Contract Type: Per-Diem

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care and support activities of daily living
Administer medications and treatments according to care plans and facility policies
Monitor and document patient status, changes, and responses to interventions
Collaborate with nurses, therapists, and care team members to ensure coordinated care
Educate patients and families about care plans, medications, and safety
Maintain accurate, timely charting in the facility electronic or paper records
Adhere to infection control standards and facility safety protocols

Qualifications

Current Licensed Practical Nurse (LPN) license — Pennsylvania licensure expected for this facility
Basic Life Support (BLS) certification preferred or obtained upon hire
Comfortable with medication administration, basic assessments, and documentation
Ability to pass background checks and meet facility immunization requirements
Strong communication skills and a collaborative, patient-first attitude

Education

LPN diploma, certificate, or equivalent practical nursing program completion

Knowledge & Skills

Skilled in patient assessments and basic nursing interventions
Medication administration and documentation accuracy
Familiarity with electronic medical records or skilled at accurate charting
Infection control practices and safe care techniques
Effective communication with patients, families, and the interdisciplinary team
Time management and prioritization in a busy clinical setting
