Roles & Responsibilities

• Deliver high-quality home health care to patients in a compassionate and professional manner
• Administer treatments and therapies according to individualized care plans
• Maintain accurate and timely patient documentation
• Collaborate with interdisciplinary teams to ensure seamless care
• Adhere to all safety and infection control protocols
• Participate in competency testing and skills checklists
• Verify and update patient health screenings and immunizations

Qualifications

• BLS certification required
• Epic experience preferred but not mandatory
• Availability for a 12-week assignment
• Commitment to 40 hours per week
• Strong communication and teamwork skills

Knowledge & Skills

• Epic electronic health record usage
• BLS protocols
• Competency testing procedures
• Health screening and immunization knowledge
