Roles & Responsibilities

• Provide comprehensive physical therapy evaluations and develop individualized treatment plans for skilled nursing facility residents
• Deliver hands‑on therapeutic interventions to improve mobility, strength, and functional independence
• Document patient progress accurately and communicate updates to interdisciplinary team
• Educate patients, families, and staff on safe mobility techniques and injury prevention
• Participate in care conferences and contribute to discharge planning

Qualifications

• Current state license as a Physical Therapist (PT) in Minnesota
• Doctor of Physical Therapy (DPT) degree or equivalent
• Ability to work in a skilled nursing facility setting
• Strong communication and teamwork skills

Education

• Doctor of Physical Therapy (DPT) degree
