Roles & Responsibilities
- Provide high-quality patient care in a home health setting
- Conduct patient assessments and develop care plans
- Perform routine visits and provide education on RT equipment
- Collaborate with interdisciplinary teams to ensure comprehensive care
Qualifications
- Current RN licensure
- BLS certification
- Valid driver's license and insurance
- Preferred experience with HCHB - HomeCare HomeBase
Education
- Associate's or Bachelor's degree in Nursing
Knowledge & Skills
- Strong clinical skills
- Excellent communication and interpersonal skills
- Ability to work independently
- Knowledge of home health care principles and practices
