Roles & Responsibilities

• Deliver comprehensive physical therapy services to residents in a skilled nursing facility and assisted living community.
• Conduct thorough assessments and develop individualized rehabilitation plans.
• Collaborate closely with interdisciplinary teams including occupational therapists, speech‑language pathologists, nurses, and physicians.
• Monitor progress, adjust treatment protocols, and document outcomes in electronic medical records.
• Educate patients and families on exercises, safety, and self‑management strategies.
• Participate in quality improvement initiatives and maintain compliance with regulatory standards.

Qualifications

• Valid Physical Therapist license in Wisconsin.
• Minimum of 0 years of experience (entry‑level acceptable).
• Strong communication and teamwork skills.
• Ability to work independently and manage a caseload efficiently.
• Commitment to providing compassionate, patient‑centered care.

Knowledge & Skills

• Physical therapy assessment and intervention techniques.
• Therapeutic exercise prescription.
• Modalities such as ultrasound, electrical stimulation, and heat/cold therapy.
• Documentation and charting in electronic health record systems.
• Interdisciplinary collaboration and care coordination.
• Patient education and discharge planning.
