Roles & Responsibilities

• Evaluate and develop individualized treatment plans for long‑term care, assisted living, and independent living residents
• Provide hands‑on therapeutic interventions to improve mobility, strength, and functional independence
• Collaborate with interdisciplinary teams including nursing, activity staff, and physicians to coordinate comprehensive care
• Document assessments, progress notes, and outcomes in compliance with regulatory standards
• Educate residents, families, and staff on injury prevention, safe transfers, and home exercise programs
• Participate in community outings, sports tournaments, and wellness activities to promote active aging

Qualifications

• Current state Physical Therapist license in New Hampshire
• Minimum 1‑2 years experience in long‑term care, assisted living, or skilled nursing environments
• Strong communication and teamwork skills
• Ability to adapt treatment plans for diverse resident needs
• Commitment to patient‑centered, compassionate care
