Shift Type: Monday-Friday, 8:00 AM - 4:30 PM

Bill Rate: $40-$4330

Roles & Responsibilities
- Evaluate and develop individualized treatment plans for long‑term care, assisted living, and independent living residents
- Provide hands‑on therapeutic interventions to improve mobility, strength, and functional independence
- Collaborate with interdisciplinary teams including nursing, activity staff, and physicians to coordinate comprehensive care
- Document assessments, progress notes, and outcomes in compliance with regulatory standards
- Educate residents, families, and staff on injury prevention, safe transfers, and home exercise programs
- Participate in community outings, sports tournaments, and wellness activities to promote active aging
Qualifications
- Current state Physical Therapist license in New Hampshire
- Minimum 1‑2 years experience in long‑term care, assisted living, or skilled nursing environments
- Strong communication and teamwork skills
- Ability to adapt treatment plans for diverse resident needs
- Commitment to patient‑centered, compassionate care

- Geriatric rehabilitation principles
- Manual therapy techniques
- Functional mobility assessments
- Electronic health record documentation
- Interdisciplinary collaboration
