Location: Santa Maria, California
Start Date: 05-24-2026
End Date: 08-23-2026
Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM (Flexible, with some weekend requirements every 3rd weekend)
Hours per Week: 40
Candidate Pay Rate: $64 per hour

Roles & Responsibilities
- Assess and treat patients in a home health setting
- Develop and implement individualized treatment plans
- Conduct OASIS assessments and document patient progress
- Collaborate with interdisciplinary teams to ensure comprehensive care
Qualifications
- Current Physical Therapist licensure
- 1 year of experience in a home health setting (preferred but not required)
- Experience with OASIS and government-mandated forms
- Basic Life Support (BLS) certification
Education
- Doctor of Physical Therapy (DPT) degree
Knowledge & Skills
- Strong clinical skills in physical therapy
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficiency in EMR systems, specifically Cerner
