Roles & Responsibilities

• Assess and develop individualized therapy plans for residents in a skilled nursing environment
• Provide hands‑on treatment to improve mobility, strength, and functional independence
• Document progress and adjust interventions based on patient response
• Collaborate with interdisciplinary team to coordinate comprehensive care
• Educate residents and families on therapeutic exercises and home‑care strategies

Qualifications

• Current state Physical Therapist license in Pennsylvania
• Passage of the National Physical Therapy Exam (NPTE)
• Strong clinical judgment and communication skills
• Ability to work independently and as part of a multidisciplinary team

Knowledge & Skills

• Geriatric rehabilitation techniques
• Skilled nursing facility regulations and documentation
• Manual therapy and therapeutic exercise modalities
• Patient safety and fall prevention strategies
