Roles & Responsibilities

• Assess and develop individualized rehabilitation plans for residents
• Provide therapeutic interventions to improve mobility and function
• Collaborate with interdisciplinary team to optimize dementia care
• Document patient progress and adjust treatment plans as needed
• Educate patients and families on therapeutic techniques and home exercise programs

Qualifications

• Current Physical Therapist license in New Hampshire
• Bachelor’s degree in Physical Therapy from an accredited program
• Minimum 1 year of clinical experience preferred
• Experience working with dementia or long‑term care populations is a plus

Knowledge & Skills

• Knowledge of geriatric and dementia care principles
• Skilled in manual therapy and therapeutic exercise techniques
• Ability to conduct functional assessments and outcome measurements
• Strong communication and teamwork skills
• Proficiency with electronic health record documentation
