Roles & Responsibilities

• Assess patient mobility and functional abilities to develop individualized therapy plans
• Design and implement evidence‑based treatment interventions to improve strength, balance, and range of motion
• Document progress notes and adjust care plans based on patient response and goals
• Collaborate with interdisciplinary team members to ensure coordinated, high‑quality patient care
• Educate patients, families, and staff on safe movement techniques and discharge planning

Qualifications

• Current state Physical Therapist license in Pennsylvania
• Completion of an accredited PT program (Doctor of Physical Therapy preferred)
• Strong communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team
• Experience in skilled nursing or acute care settings is a plus

Knowledge & Skills

• Knowledge of musculoskeletal and neurological rehabilitation techniques
• Skilled in gait training, therapeutic exercise, and manual therapy
• Proficient in patient assessment and outcome measurement tools
• Familiarity with electronic health record documentation
