Roles & Responsibilities

Conduct physical therapy assessments and interventions for patients in home health settings
Develop and implement treatment plans for patients with various conditions, including orthopedic and geriatric cases
Collaborate with nursing staff and other healthcare professionals to ensure comprehensive care
Perform SOC visits, especially for orthopedic patients when nursing is not ordered

Qualifications

2 years of experience in Physical Therapy, with Home Health experience preferred
BLS certification required
Physical Therapy licensure in California
Experience with EMR systems, preferably HCHB - HomeCare HomeBase

Education

Physical Therapy degree from an accredited program

Knowledge & Skills

Physical Therapy assessment and intervention techniques
Home Health care principles and practices
EMR system proficiency (HCHB - HomeCare HomeBase)
Strong communication and interpersonal skills
