Roles & Responsibilities

• Deliver high‑quality physical therapy services to residents in a skilled nursing facility.
• Conduct comprehensive assessments and develop individualized treatment plans.
• Collaborate with interdisciplinary team to optimize patient outcomes.
• Document progress and maintain accurate patient records.
• Educate patients and families on therapeutic exercises and home care.

Qualifications

• Current Pennsylvania Physical Therapist license.
• Minimum of 1 year of experience in skilled nursing or long‑term care setting.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.

Knowledge & Skills

• Proficiency in therapeutic exercise prescription.
• Manual therapy techniques.
• Gait and balance training.
• Pain management strategies.
• Documentation using electronic health records.
