Roles & Responsibilities

• Provide therapeutic interventions to residents with dementia, ensuring individualized care plans are met
• Develop and implement evidence‑based OT treatment plans tailored to each resident’s needs
• Collaborate closely with interdisciplinary team members to coordinate care and optimize outcomes
• Document progress, outcomes, and treatment plans accurately and timely
• Assist in the rehabilitation of patients across all functional domains
• Advocate for residents’ needs and promote a supportive, person‑centered environment

Qualifications

• Valid Occupational Therapist license in New Hampshire
• Bachelor’s or Master’s degree in Occupational Therapy (or equivalent)
• Experience working with dementia or geriatric populations preferred
• Strong communication and teamwork skills
• Commitment to continuous learning and professional development

Knowledge & Skills

• Knowledge of OT assessment and intervention techniques
• Familiarity with dementia care best practices
• Proficiency in documentation and electronic health record systems
• Strong organizational and time‑management abilities
• Ability to adapt interventions to individual resident needs
