Location: Lebanon, New Hampshire
Start Date: 09-30-2026
Contract Type: Perm
Shift Type: Tuesday-Saturday, 8:00 AM - 4:30 PM (or Sunday-Thursday, 8:00 AM - 4:30 PM)
Hours per Week: 35
Bill Rate: $37-$39.50/hr

- 10% Salary Placement fee
- 35 hours
- Tues-Sat or Sun- Thurs schedule
- No weekend rotation

Roles & Responsibilities
- Assess resident functional abilities and develop individualized occupational therapy plans
- Implement therapeutic interventions to improve daily living skills and mobility
- Document progress notes, treatment outcomes, and adjust care plans as needed
- Collaborate with interdisciplinary team members to coordinate comprehensive patient care
- Educate residents, families, and staff on adaptive techniques and equipment
- Participate in discharge planning and provide recommendations for continued therapy
Qualifications
- Current New Hampshire Occupational Therapist license
- BLS certification preferred
- Entry‑level to 2 years of OT experience in skilled nursing or rehabilitation settings
- Strong communication and teamwork skills
- Ability to prioritize and manage a caseload independently
Knowledge & Skills
- Knowledge of geriatric rehabilitation and functional assessment tools
- Proficiency in therapeutic exercise, ADL training, and adaptive equipment use
- Understanding of Medicare/Medicaid documentation requirements
- Effective documentation and electronic health record navigation
- Problem‑solving and critical‑thinking abilities
