Roles & Responsibilities

• Conduct comprehensive functional assessments to determine patients' occupational therapy needs
• Develop and implement individualized treatment plans that promote independence and improve daily living skills
• Document progress notes and adjust interventions based on patient response
• Collaborate with interdisciplinary team members to coordinate holistic care
• Educate patients and families on adaptive techniques and home modifications

Qualifications

• Current state licensure as an Occupational Therapist
• Completion of an accredited OT program (MS or BS)
• Strong clinical judgment and excellent communication skills
• Ability to work independently in a skilled nursing facility

Knowledge & Skills

• Knowledge of post‑acute rehabilitation protocols
• Proficiency in therapeutic equipment and adaptive devices
• Skilled in outcome measurement and documentation
• Effective patient education techniques
