Roles & Responsibilities

• Screen and evaluate patients with cognitive and/or ADL physical functioning disorders
• Develop and implement individualized plans of care using standardized assessments and evidence-based practice
• Provide rehabilitative, skilled, and medically necessary treatment interventions including ADL retraining, therapeutic activities, and exercises
• Monitor patient progress and adjust treatments as needed, documenting all regulatory and clinically required information
• Communicate effectively with patients, families, caregivers, and the healthcare team regarding progress and treatment plans
• Supervise occupational therapist assistants, rehabilitation aides, and students as needed
• Participate in patient care conferences, utilization review meetings, and required trainings

Qualifications

• 1+ year of Occupational Therapy experience required (new grads welcome with Level II fieldwork experience)
• Long Term Care or Rehab experience required
• Active Occupational Therapy license in Texas
• Ability to obtain necessary DME or supplies per regulatory requirements
