Contract Type: Per-Diem

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate hands-on nursing care to residents and patients
Administer medications and treatments accurately and safely
Monitor, record and report changes in patient condition and vital signs
Collaborate with RNs and interdisciplinary team members to implement care plans
Maintain clear, timely documentation in the medical record
Communicate effectively with residents, families and the care team
Support infection control and quality improvement initiatives

Qualifications

Active LPN license in Pennsylvania (required)
Graduation from an accredited practical nursing program
Current BLS/CPR certification (or willingness to obtain)
Comfortable with medication administration, patient assessments and documentation
Strong communication and teamwork skills; long-term care experience preferred
Up-to-date immunizations and ability to complete facility onboarding

Education

Completion of an accredited Practical Nursing / LPN program
High school diploma or GED

Knowledge & Skills

Medication administration and basic clinical procedures
Resident assessment and care planning fundamentals
Documentation best practices and electronic medical record familiarity
Infection prevention and safety protocols
Strong interpersonal skills and patient-focused communication
Time management and the ability to prioritize in a busy setting
