Roles & Responsibilities
- Provide compassionate hands-on nursing care to residents and patients
- Administer medications and treatments accurately and safely
- Monitor, record and report changes in patient condition and vital signs
- Collaborate with RNs and interdisciplinary team members to implement care plans
- Maintain clear, timely documentation in the medical record
- Communicate effectively with residents, families and the care team
- Support infection control and quality improvement initiatives
Qualifications
- Active LPN license in Pennsylvania (required)
- Graduation from an accredited practical nursing program
- Current BLS/CPR certification (or willingness to obtain)
- Comfortable with medication administration, patient assessments and documentation
- Strong communication and teamwork skills; long-term care experience preferred
- Up-to-date immunizations and ability to complete facility onboarding
Education
- Completion of an accredited Practical Nursing / LPN program
- High school diploma or GED
Knowledge & Skills
- Medication administration and basic clinical procedures
- Resident assessment and care planning fundamentals
- Documentation best practices and electronic medical record familiarity
- Infection prevention and safety protocols
- Strong interpersonal skills and patient-focused communication
- Time management and the ability to prioritize in a busy setting
