Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents consistent with LPN/RN scope of practice
• Perform thorough patient assessments and monitor changes in condition, communicating findings to the care team
• Administer medications and treatments accurately and document all care in the medical record
• Collaborate with interdisciplinary team members to develop and update individualized care plans
• Respond promptly to overnight events and emergencies, prioritizing resident safety and comfort
• Educate residents and families about care plans and interventions in a clear, respectful way

Qualifications

• Current and active LPN or RN license (eligible to practice in West Virginia)
• Basic Life Support (BLS) certification required; advanced certifications preferred where applicable
• Clinical experience in long-term care or skilled nursing preferred
• Strong documentation and electronic charting skills
• Reliable, professional, and able to meet facility attendance expectations
• Ability to pass facility credentialing and background checks

Education

• Licensed Practical Nurse: Graduation from an accredited LPN program
• Registered Nurse: Associate's Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred

Knowledge & Skills

• Skilled in patient assessment, medication administration, and wound care basics
• Comfortable with overnight monitoring, triage, and escalation of care
• Good clinical judgment and the ability to prioritize tasks in a busy environment
• Strong communication skills for handoffs and interdisciplinary collaboration
• Familiarity with electronic health records and accurate charting practices
• Team-oriented mindset with a focus on resident-centered care
