Roles & Responsibilities

• Provide compassionate, accurate bedside care to assigned patients
• Administer medications and document treatments according to policies
• Monitor patient status and report changes to the care team promptly
• Perform routine assessments and assist with care plans
• Collaborate with RNs, providers, and support staff to deliver seamless care
• Maintain clear, timely documentation in the medical record

Qualifications

• Active LPN/LVN license (New York) — required
• Current Basic Life Support (BLS) certification preferred or obtainable upon hire
• Proven comfort with medication administration and basic nursing procedures
• Strong communication skills and ability to work well on a team
• Reliable, professional attitude and a patient-first mindset

Education

• Graduate of an accredited Practical Nursing (LPN/LVN) program
• High school diploma or equivalent

Knowledge & Skills

• Practical nursing skills including vital signs, wound care, and med administration
• Solid charting and documentation practices
• Ability to prioritize tasks in a fast-paced clinical setting
• Familiarity with electronic health records (EHR) systems
• Good clinical judgment and patient advocacy
