Roles & Responsibilities

• Perform comprehensive assessments of patients' needs and coordinate individualized care plans
• Provide skilled pain and symptom management consistent with the hospice plan of care
• Educate and support patients and families regarding the disease process, self-care techniques, and end-of-life issues
• Collaborate with the interdisciplinary team including physicians, social workers, chaplains, and volunteers
• Maintain accurate and timely documentation in compliance with state, federal, and organizational policies

Qualifications

• Current Registered Nurse (RN) license in Utah or compact state
• Minimum of 3 years nursing experience, preferably in hospice, home health, or palliative care
• Current BLS, ACLS, PALS, and NRP certifications
• Valid driver's license and reliable transportation for home visits
• Strong clinical assessment and communication skills
• Compassionate demeanor with ability to support patients and families during difficult times

Education

• Associate or Bachelor's degree in Nursing
• Current RN license in Utah or compact state

Knowledge & Skills

• Hospice care and end-of-life support
• Patient assessment and pain management
• Symptom management techniques
• HospiceSoft/Careficient EMR experience preferred
• Interdisciplinary team collaboration
• Accurate documentation and compliance
