Roles & Responsibilities

• Provide compassionate daily nursing care to residents, helping with activities of daily living and promoting dignity
• Administer medications accurately and document administration in the chart
• Assess and monitor resident condition, recording vital signs and reporting changes promptly
• Collaborate with nurses, therapists, and care staff to implement care plans that improve outcomes
• Maintain clear, timely documentation and charting to support continuity of care
• Support a safe, clean environment and follow infection control protocols

Qualifications

• Current Licensed Practical Nurse (LPN) license (Massachusetts) required
• Graduate of an accredited Practical Nursing program (LPN/LVN)
• Current CPR/BLS certification preferred
• Strong communication skills and a team-first attitude
• Prior experience in long-term care or skilled nursing is a plus

Education

• Graduate of an accredited Practical Nursing program (LPN/LVN diploma)

Knowledge & Skills

• Medication administration and basic medication safety
• Skilled in vital signs, patient observation, and documentation
• Basic wound care and dressing changes
• Infection control practices and resident safety
• Comfortable with electronic charting and care plans
• Empathy, patience, and strong interpersonal skills
