Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in a long-term care and rehab setting
• Administer medications and treatments accurately and on time according to care plans
• Monitor, assess and document vital signs, symptoms and changes in condition
• Collaborate with RNs, therapists and support staff to deliver coordinated care
• Assist with activities of daily living and support resident comfort and dignity
• Communicate clearly with patients, families and the care team about care goals and progress
• Maintain accurate and timely documentation in the medical record and follow facility protocols

Qualifications

• Active LPN license in Massachusetts required
• Current CPR/BLS certification (or willingness to obtain upon hire)
• Comfortable working night shifts in a long-term care/rehabilitation environment
• Strong clinical judgment, attention to detail, and compassionate bedside manner
• Previous experience in long-term care or rehab settings preferred

Education

• LPN diploma or vocational nursing certificate
• High school diploma or GED

Knowledge & Skills

• Medication administration and accurate charting
• Basic wound care and monitoring of skin integrity
• Vital sign assessment and recognition of acute changes
• Infection control practices and safe patient handling
• Clear communication and teamwork in a multidisciplinary setting
• Time management and prioritization during busy night shifts
