Roles & Responsibilities

• Administer medications and treatments to patients in a safe and compassionate manner
• Monitor patient vital signs and report changes to the healthcare team
• Collaborate with nurses, physicians, and allied health professionals to develop and implement care plans
• Maintain accurate and timely patient documentation in electronic health records
• Educate patients and families on health conditions, medications, and self-care practices
• Assist with daily living activities and support patient comfort and dignity
• Adhere to infection control protocols and safety standards
• Participate in quality improvement initiatives and team meetings

Qualifications

• Valid LPN license in Vermont
• PCC (Patient Care Coordinator) training or certification
• Strong communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary team
• Flexibility to travel and adapt to new environments
• Commitment to providing high-quality patient care
