Shift Type: Monday-Friday, 6:00 AM - 2:30 PM
Hours per Week: 40
Candidate Pay Rate: $42 per hour

Roles & Responsibilities
- Administer medications and treatments to patients in a safe and compassionate manner
- Monitor patient vital signs and report changes to the healthcare team
- Collaborate with nurses, physicians, and allied health professionals to develop and implement care plans
- Maintain accurate and timely patient documentation in electronic health records
- Educate patients and families on health conditions, medications, and self-care practices
- Assist with daily living activities and support patient comfort and dignity
- Adhere to infection control protocols and safety standards
- Participate in quality improvement initiatives and team meetings
Qualifications
- Valid LPN license in Vermont
- PCC (Patient Care Coordinator) training or certification
- Strong communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Flexibility to travel and adapt to new environments
- Commitment to providing high-quality patient care
