Roles & Responsibilities

• Coordinate care for pre-transplant, waitlist, and post-transplant solid organ patients with compassion and attention to detail
• Manage patient documentation and charting using Meditech, TransChart, and PatientKeeper
• Perform clinic rooming, patient assessments, and basic clinical interventions during clinic days
• Participate in WebEx meetings and interdisciplinary case discussions as needed
• Collaborate with transplant surgeons, coordinators, and support staff to ensure seamless patient care
• Maintain professional, empathetic communication to support complex patient psychosocial needs
• Jump in to help the team wherever needed and adapt to changing patient assignments

Qualifications

• Active RN license (required)
• BLS certification (required)
• Minimum 1 year of RN clinical experience (as noted)
• Preferred: 2+ years of transplant experience
• Preferred experience with UNOS and OPO processes
• Comfortable and proficient with EMR systems (Meditech, TransChart, PatientKeeper)
• Willingness to be flexible in patient assignments and work in an outpatient transplant environment

Knowledge & Skills

• Proficiency with Meditech, TransChart and PatientKeeper EMRs
• Strong Microsoft Office skills for desk-based documentation and communication
• Ability to perform thorough patient assessments and clinical documentation
• Familiarity with transplant workflows, UNOS terminology, and OPO coordination (preferred)
• Excellent communication, teamwork, and time-management skills
• Comfort with virtual meetings (WebEx) and remote collaboration tools
