Roles & Responsibilities

• Coordinate comprehensive care plans by collaborating with patients, families, and clinical teams
• Perform timely discharge planning to ensure safe transitions and reduce readmissions
• Advocate for patients by addressing barriers to care and connecting them to community resources
• Document case management activities clearly and accurately to support continuity of care
• Communicate proactively with interdisciplinary teams to streamline patient care
• Monitor patient progress and adjust care plans to achieve optimal outcomes

Qualifications

• Active Registered Nurse (RN) license — Florida RN or multistate compact RN preferred
• Minimum 2 years of nursing experience (case management experience strongly preferred)
• Knowledge of case management principles and care coordination best practices
• Strong clinical assessment, documentation, and time-management skills
• Excellent communication and patient advocacy skills
• Current BLS (Basic Life Support) certification; other certifications (CCM, ACM-RN) a plus

Education

• Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
• BSN preferred but all qualified RNs are encouraged to apply

• Care coordination and discharge planning expertise
• Patient advocacy and resource referral know-how
• Clinical assessment and critical thinking
• Strong documentation skills and attention to detail
• Ability to collaborate across interdisciplinary teams
• Familiarity with healthcare regulations and quality standards
