Roles & Responsibilities

• Evaluate and assess patients’ functional mobility and develop individualized treatment plans
• Provide hands-on therapy, gait training, balance and strength work to help patients progress
• Collaborate with nurses, physicians and the rehab team to coordinate care goals
• Document patient progress clearly and timely in the facility’s medical record system
• Educate patients and families on home programs and safe functional strategies
• Participate in discharge planning to support smooth transitions and positive outcomes

Qualifications

• Current licensure as a Physical Therapist in Pennsylvania or eligibility to obtain license
• Degree in Physical Therapy (DPT or equivalent) and relevant clinical training
• BLS/CPR certification (or willingness to obtain prior to start)
• Comfort working in an inpatient/rehab setting and collaborating with interdisciplinary teams
• Ability to work Monday–Friday daytime schedule and meet documentation requirements
• Pass required background and health screenings

Education

• Doctor of Physical Therapy (DPT) preferred, or Bachelor's/Master’s in Physical Therapy
• Relevant clinical internships or acute/rehab experience preferred

Knowledge & Skills

• Skilled in assessment, treatment planning and therapeutic interventions for adult rehab
• Experience with gait training, transfers, balance and functional mobility training
• Strong clinical reasoning and progress-tracking abilities
• Effective interdisciplinary communication and patient/family education skills
• Familiarity with electronic medical records and timely documentation practices
