Location: Santa Maria, California

Start Date: 05-25-2026

End Date: 08-24-2026

Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM (Flexible, with some weekend requirements every 3rd weekend)

Hours per Week: 40

Candidate Pay Rate: $64 per hour

Roles & Responsibilities

Assess and treat patients in a home health setting
Develop and implement individualized treatment plans
Conduct evaluations and progress assessments
Collaborate with other healthcare professionals
Maintain accurate and up-to-date patient records

Qualifications

Current Physical Therapist license
1+ year of experience in a home health setting (preferred)
Experience with OASIS (government-mandated form)
Basic Life Support (BLS) certification

Education

Doctor of Physical Therapy (DPT) degree

Knowledge & Skills

Strong clinical skills in physical therapy
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
Proficiency in EMR systems (Cerner)
Knowledge of government-mandated forms (OASIS)
