Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Conduct comprehensive patient assessments and develop individualized treatment plans
Provide skilled physical therapy interventions to improve mobility, strength, and function
Perform OASIS assessments and Start of Care visits for home health patients
Collaborate with the healthcare team to coordinate patient care and document progress
Educate patients and families on home exercise programs and fall prevention strategies

Qualifications

Active Physical Therapy license in Indiana
BLS certification required
Experience with OASIS assessments and Start of Care documentation
Strong interpersonal and communication skills
Ability to work independently in a home health setting

Knowledge & Skills

Knowledge of orthopedic and geriatric physical therapy
Skilled in patient evaluation and treatment planning
Proficient with electronic health records and tablet-based documentation
Understanding of home health regulations and point systems
