Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM (rotating Saturdays once a month)

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Assess, evaluate, and update therapeutic and functional status for patients in their home environment
Develop and implement individualized physical therapy plans of care in collaboration with physicians
Instruct patients and families on therapeutic exercises and use of adaptive equipment
Supervise home health aides and participate in case conferences to coordinate care
Document patient progress and communicate changes to the healthcare team
Travel to patient homes across Yolo County, providing essential rehabilitation services

Qualifications

Minimum 2 years of experience as a Physical Therapist in acute care, home health, or rehab setting
Active and unrestricted Physical Therapist license in California
Current BLS certification (AHA or equivalent)
Excellent communication and organizational skills
Ability to work independently and travel to multiple patient locations daily

Knowledge & Skills

Expertise in physical therapy assessment and treatment modalities
Knowledge of Medicare, Title 22, and home health regulations
Strong clinical reasoning and problem-solving abilities
Proficiency in documenting patient care and using electronic health records
