Roles & Responsibilities

• Provide exceptional home health physical therapy services to patients in and around Valley City
• Develop and implement individualized treatment plans to help patients recover from surgeries, falls, and other conditions
• Educate patients and their families on therapeutic exercises and strategies for independent living
• Collaborate with interdisciplinary team members to ensure comprehensive patient care
• Maintain accurate documentation using HomeCare/HomeBase EMR system
• Travel within a 60-mile radius to deliver care in patients' homes

Qualifications

• Current North Dakota Physical Therapy license in good standing
• Basic Life Support (BLS) certification from the American Heart Association
• Valid driver's license and auto insurance required
• Minimum 1 year of physical therapy experience, preferably in home health
• Previous travel experience not required but welcomed

Knowledge & Skills

• Strong clinical skills in physical therapy assessment and treatment
• Familiarity with home health documentation and HomeCare/HomeBase EMR is a plus
• Excellent communication and patient education abilities
• Ability to work independently and manage a daily caseload of 5-7 patients
• Comfort with driving and providing care in a home setting
