Roles & Responsibilities

Deliver exceptional physical therapy to patients in their homes across vibrant Northern California territories
Conduct comprehensive OASIS assessments, evaluations, and treatments to improve mobility and quality of life
Collaborate with healthcare teams to create personalized care plans that make real differences
Manage documentation and patient communications with efficiency and care
Travel to scenic areas like Napa Valley while providing impactful home health services

Qualifications

Active CA PT License
BLS certification
Valid Driver's License with Auto insurance and registration
Willingness to see COVID+ patients and cover weekends if needed

Education

Knowledge & Skills

Expertise in home health therapy and OASIS documentation
Strong patient assessment and evaluation skills
Comfortable with travel and flexible scheduling
Proficiency in productivity-based visit planning
