Roles & Responsibilities

• Provide skilled physical therapy services to patients in their homes, helping them regain independence and improve quality of life
• Develop and implement individualized treatment plans based on patient needs and goals
• Conduct comprehensive evaluations including SOC visits for orthopedic patients when nursing is not ordered
• Collaborate with interdisciplinary team members to ensure coordinated care
• Document patient progress using HCHB EMR in a timely and accurate manner
• Educate patients and families on home exercise programs and safety strategies

Qualifications

• Active Physical Therapist license in California (or ability to obtain before start)
• Current BLS certification from the American Heart Association
• Minimum 2 years of experience in Physical Therapy (home health experience preferred; travelers with less experience considered if they have home health experience)
• Willingness to travel within Tehama County service area (approximately 40 miles per day)

Knowledge & Skills

• Expertise in treating orthopedic and geriatric patients with multiple co-morbidities (CHF, COPD, joint replacements)
• Proficiency in HCHB (HomeCare HomeBase) EMR system
• Strong clinical reasoning and assessment skills
• Excellent communication and patient education abilities
• Ability to work independently and manage time effectively during home visits
