Roles & Responsibilities

Conduct physical therapy sessions for patients in their homes
Perform SOC visits, especially for ortho patients
Utilize EMR system (HCHB - HomeCare HomeBase) on a provided tablet
Manage a caseload of approximately 5 visits per day

Qualifications

2 years of experience in Physical Therapy (will consider travelers with less experience and Home Health experience)
BLS certification required
Experience with Home Health and Physical Therapy

Education

Physical Therapy degree

Knowledge & Skills

Physical Therapy skills
Home Health experience
BLS certification
EMR system usage (HCHB - HomeCare HomeBase)
