Roles & Responsibilities

• Provide direct physical therapy care to home health patients, delivering compassionate, individualized treatment
• Collaborate with interdisciplinary team members to develop and maintain comprehensive plans of care
• Perform patient assessments, document progress, and ensure accurate OASIS completion
• Recommend, fit, and instruct patients on rehabilitation equipment including splints, orthoses, prostheses, wheelchairs, and ADL devices
• Use therapeutic modalities safely and effectively to support functional recovery
• Educate patients and caregivers on home exercise programs, safety, and fall prevention
• Prioritize independent decision-making and efficient time management while working in the community setting

Qualifications

• Massachusetts Physical Therapist license (must be uploaded with profile)
• Home Health Certification (required; non-negotiable)
• Active American Heart Association BLS (AHA)
• Minimum 2 years combined acute nursing/hospital-base and home care experience
• Valid driver's license and reliable personal vehicle for patient visits
• Ability to work independently with minimal supervision
• Current supervisory reference from within the past two years
• Traveler must not reside within 50 miles of the facility (per facility policy)

Knowledge & Skills

• Proficiency with OASIS documentation and home health outcome measures
• Strong skills in plan of care development and patient-centered goal setting
• Experience with rehabilitation equipment: splints, orthoses, prostheses, wheelchairs, ADL equipment
• Familiarity with therapeutic modalities and safe application
• Solid patient assessment, clinical reasoning, and discharge planning abilities
• Excellent communication skills for patient education and interdisciplinary collaboration
• Comfort managing a home-based caseload and adapting interventions in varied environments
