Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekends

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Conduct comprehensive evaluations and develop individualized treatment plans
Perform start of care visits, recertifications, and discharges for home health patients
Provide skilled physical therapy interventions including therapeutic exercises and modalities
Collaborate with a multidisciplinary team to ensure coordinated patient care
Complete timely and accurate documentation using provided tablet

Qualifications

Active Physical Therapist license in Kentucky (or compact eligible)
BLS certification required
No new grads preferred; at least 1 year of experience in home health or related setting
Comfortable with rotating weekend and holiday schedules

Knowledge & Skills

Expertise in treating common home health diagnoses: CHF, orthopedic conditions, pneumonia
Ability to work independently and manage a caseload of 6.5 visits per day
Strong communication and documentation skills
Familiarity with home health regulations and OASIS documentation
