Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 4:00 PM or 9:00 AM - 5:00 PM, weekends as needed

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Deliver compassionate home health therapy to patients in their own homes, making a real difference in their daily lives
Develop personalized treatment plans that empower patients to regain independence and mobility
Collaborate with interdisciplinary teams to ensure seamless, holistic care
Educate patients and families on exercises and self-care techniques for lasting results
Document progress with precision using systems like Epic to track every win

Qualifications

Active Physical Therapy license in good standing
BLS certification
Experience in home health settings preferred
Epic experience a plus, but we'll get you up to speed

Education

Graduate of an accredited Physical Therapy program
State PT licensure required

Knowledge & Skills

Strong clinical assessment and manual therapy skills
Excellent patient communication and empathy
Proficiency with electronic health records like Epic
Ability to work independently in home environments
