Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct physical therapy assessments and treatments for patients in Home Health and Hospice settings
Develop and implement individualized treatment plans
Collaborate with other healthcare professionals to ensure comprehensive care
Perform start of care, recerts, and resumption of care visits

Qualifications

Current ND Physical Therapy License
BLS certification
Home Health experience preferred
OASIS training preferred

Education

Physical Therapy degree from an accredited program

Knowledge & Skills

Physical therapy techniques and best practices
Home Health and Hospice operations
OASIS data collection and reporting
Effective communication and interpersonal skills
