Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 4:30 PM

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Provide skilled physical therapy services to residents in a skilled nursing facility
Develop and implement individualized treatment plans to improve mobility, strength, and function
Collaborate with an interdisciplinary team to ensure comprehensive patient care
Educate patients and families on therapeutic exercises and home programs
Document patient progress and adjust treatments as needed
Promote a positive and healing environment for optimal recovery

Qualifications

Valid Physical Therapist license in Connecticut (or compact license eligible)
Graduate from an accredited physical therapy program (Master's or Doctorate)
Minimum 1 year of experience in a skilled nursing or rehab setting
Current BLS certification from the American Heart Association
Strong clinical reasoning and problem-solving skills
Excellent communication and teamwork abilities

Knowledge & Skills

Expertise in evaluating and treating orthopedic, neurological, and geriatric conditions
Proficient in electronic medical records and documentation
Ability to work independently while being a collaborative team player
Compassionate and patient-centered approach to care
Time management skills to handle a diverse caseload
