Contract Type: Travel Contract
Shift Type: Monday-Friday, 7:00 AM - 3:00 PM (Day shift, 8-hour days; flexible start between 6:00 AM and 10:00 AM as arranged with DOR)
Hours per Week: 40
Candidate Pay Rate: $58 per hour

Roles & Responsibilities
- Screen and evaluate patients with cognitive and/or ADL physical functioning disorders to create individualized plans of care
- Use standardized assessments and evidence-based practice to guide clinical interventions
- Deliver rehabilitative, skilled, and medically necessary treatments including ADL retraining, therapeutic activities, therapeutic exercises, and modalities
- Assess patient progress, adjust treatments, and evaluate outcomes to ensure effective care
- Document treatment, treatment minutes, and regulatory/clinical information accurately in the electronic record
- Provide consultation and counseling to patients, families, caregivers, and other service providers
- Coordinate care with the interdisciplinary team and participate in care conferences and utilization review meetings
- Prepare comprehensive discharge summaries and ensure continuity of care
Qualifications
- Active Occupational Therapy license at time of submission (Oklahoma license required)
- Minimum 1+ years of Occupational Therapy experience (LTC or Rehab experience required)
- New graduates considered if they have completed Level II fieldwork (Level I observation alone is not sufficient)
- Ability to supervise and support OT assistants, rehabilitation aides, and students
- Strong documentation skills and adherence to NBCOT and state practice acts, code of conduct, and ethics
- Comfort with obtaining necessary DME and working within regulatory and clinical practice requirements
Knowledge & Skills
- Patient assessment and evaluation for cognitive and ADL physical functioning
- ADL retraining and therapeutic activities/exercises
- Use of standardized assessments and evidence-based interventions
- Clinical documentation and electronic health record proficiency
- Care coordination and interdisciplinary communication
- Discharge planning and utilization review familiarity
- Ability to supervise OT assistants and mentor students
- DME procurement and patient/caregiver education
