Roles & Responsibilities

• Screen and evaluate patients with cognitive and/or ADL physical functioning disorders to create individualized plans of care
• Use standardized assessments and evidence-based practice to guide clinical interventions
• Deliver rehabilitative, skilled, and medically necessary treatments including ADL retraining, therapeutic activities, therapeutic exercises, and modalities
• Assess patient progress, adjust treatments, and evaluate outcomes to ensure effective care
• Document treatment, treatment minutes, and regulatory/clinical information accurately in the electronic record
• Provide consultation and counseling to patients, families, caregivers, and other service providers
• Coordinate care with the interdisciplinary team and participate in care conferences and utilization review meetings
• Prepare comprehensive discharge summaries and ensure continuity of care

Qualifications

• Active Occupational Therapy license at time of submission (Oklahoma license required)
• Minimum 1+ years of Occupational Therapy experience (LTC or Rehab experience required)
• New graduates considered if they have completed Level II fieldwork (Level I observation alone is not sufficient)
• Ability to supervise and support OT assistants, rehabilitation aides, and students
• Strong documentation skills and adherence to NBCOT and state practice acts, code of conduct, and ethics
• Comfort with obtaining necessary DME and working within regulatory and clinical practice requirements

Knowledge & Skills

• Patient assessment and evaluation for cognitive and ADL physical functioning
• ADL retraining and therapeutic activities/exercises

• Clinical documentation and electronic health record proficiency
• Care coordination and interdisciplinary communication
• Discharge planning and utilization review familiarity
• Ability to supervise OT assistants and mentor students
• DME procurement and patient/caregiver education
